Vehicle: 2024 Chevrolet Blazer VIN: [XXX] Component: Engine Cooling System (Radiator) Description of Incident: On XXX the vehicle was brought in for a routine maintenance service at approximately 20,000 miles. During the inspection, technicians at Hamilton Chevrolet in Warren, Michigan discovered a premature radiator leak. The servicing technician noted that this is a common failure on modern Blazers due to a manufacturing material mismatch, specifically where the plastic end tanks meet the aluminum radiator core. This structural component failure poses a severe safety risk. A rapid loss of engine coolant can cause immediate engine overheating, thermal management failure, and a sudden loss of vehicle propulsion while driving at highway speeds, significantly increasing the risk of a crash. There was a wreck up ahead, and two tow trucks with their emergency lights flashing were blocking the left two lanes. Traffic was stop-and-go due to the existing wreck. I was traveling in the lane to the right of the tow trucks as I approached. I stopped about 1–2 car lengths behind them. When traffic started to move, I began to move forward and advanced about a car length when the automatic emergency braking (AEB) activated and slammed on the brakes, even though the path in front of me was clear. The person behind me rear-ended me. Luckily, no one was physically hurt, but there is damage to our car that now has to be repaired, along with all the headache that brings. If this had happened at higher speeds, it could have resulted in serious injuries. My family’s safety and the safety of others are definitely at risk because of this phantom braking. The contact owns a 2024 Chevrolet Blazer. The contact stated that while driving at an undisclosed speed, the vehicle experienced phantom braking on four separate occasions. The Forward Collision Avoidance warning light was illuminated. The vehicle was taken to the dealer, but the cause of the failure could not be determined. The vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 40,000.

Sudden catastrophic drive train failure while driving. Car stopped instantly and was totally immobilized. It had to be dragged onto a flatbed tow truck. It is at dealership now for evaluation.
